Every AI system we ship answers this before it ships.
Approval mode is the default. A new agent proposes; a person disposes. Nothing reaches a patient, a payer, or a customer without a named human clearing it.
Escalations pause. They don’t proceed. When an agent needs a human and doesn’t get one, the work parks in a resumable state and waits — through a weekend, a holiday, or a Friday-to-Saturday-night window. It never guesses in order to finish.
Reversible and irreversible actions are separated in code. Drafting a note is reversible. Submitting a claim is not. Irreversible actions stay gated permanently unless you decide otherwise, in writing.
You loosen the gates, not us. As a workflow proves itself, you decide which steps run unattended. Some clients keep approvals on everything. That’s a valid configuration, not a failure.
Someone independent checks the work
The most common way an AI system fails isn’t a wrong answer. It’s reporting success on work it never did — a file that was never written, an API call that returned empty, a task marked complete after the escalation window expired.
A model checking its own output shares its own blind spots. So every system we build is reviewed by a second one: a different model family, a clean context, given the event log and the written policy — with no knowledge of what the first system intended.
It runs inside your boundary. Same tenant, same BAA, no new sub-processors. The auditor reads structure — task type, gate state, tool returns, timing, whether the artifact actually exists — not patient content. You get the report. So does your compliance officer.
